Before Your Appointment
Where do I call for an appointment?
Call the clinic directly to schedule your appointment.
How do I prepare for the office visit?
It’s always wise to confirm that your insurance will cover the care you’ll receive before you schedule a visit. If your plan requires a referral, make sure to get one from your physician. A few days before your appointment, a pre-registration representative will call to pre-register you, so there's less paperwork to complete on the day of your appointment. You should, nevertheless, plan on arriving for your appointment 30 minutes early to fill out forms that cannot be completed over the phone.
What should I bring with me to the office visit?
Bring:
· Pertinent x-rays, tests and medical reports
· A list of any prescription and non-prescription medications you are taking
· The phone number of your pharmacy
· Your insurance card
· The name and phone number of your primary care doctor and clinic
· Cash, check, or credit card for any co-pay your insurance requires

How long will my appointment last?
Although this varies by physician and treatment, it’s wise to allow at least 1 hour for the visit.
What parking is available?
The parking lot adjacent to our Burnsville office is free. The ramp connected to Fairview Southdale Hospital by a skyway across France Avenue offers the best parking for our Edina office. The hourly rate for this ramp is $2.00.
What forms of insurance or payment do you accept?
We participate with many HMO’s, PPO’s and Care Systems, and we will do everything we can to help you get the most from your medical insurance. A pre-registration representative will call you before your first appointment to collect your insurance information and pre-register you. Please bring your insurance cards to your first appointment.
Be sure to check your plan coverage and network listings before you visit. If your policy requires a referral from your primary care physician, make sure you get that authorization or your insurance company may refuse to pay for your care. Failure to obtain an authorization could mean that you would be responsible for payment of your medical costs. It is also crucial that you verify that the hospital location where you surgery will be done is in your insurance company’s network and that your policy covers the specific type of surgery you are having. If your coverage requires a co-payment please bring a means of payment (cash, check, MasterCard or Visa) to your appointment or a service charge may be added to your account.
Our Administrative Office can provide you with an estimate of your hernia surgery costs. (Since coverages vary, the figure may not be precise.) We do offer financing options to cover your portion of the cost, and we accept cash, checks, MasterCard or Visa. In some cases, you may be asked to pay for all or a portion of your surgery in advance. We will notify you if prepayment for your surgery is required.
We also participate in the Medicare program and our Administrative Office can submit your claim to Medicare and any supplemental insurance carriers.

Before Your Surgery:
Should I check with my insurance carrier to confirm my coverage?
Yes. It’s a good idea to make sure your insurance benefits cover the specific location and type of surgery you are having.
How long do I have to wait until my surgery?
Scheduling does vary depending on diagnosis urgency. Rest assured that we will work with you to schedule your surgery in as timely a manner is appropriate to your procedure.
Who can help me complete any required disability or insurance forms?
Make sure to bring or fax your forms to the office where you had your consultation. For the fax number of either of our clinics click here. Submitted forms are usually ready for pick up within 10 - 14 business days.
Is a pre-surgery physical required?
Most procedures require a pre-operative history and physical. If required, you will need to schedule an appointment with your primary care physician.
After Your Surgery:
Who do I call if I have questions about my recovery process?
It is common to have post-operative questions. A qualified staff member at your MHC office will gladly answer any concern you have as quickly as possible. For the phone number of your MHC office click here.

When should I call my doctor?
Call your MHC office if your recovery is not progressing as discussed with your physician. We are always available to answer your questions. For the phone number of your MHC office click here.
Who do I call after hours?
An on-call physician is always available for urgent concerns at your SCPA office phone number. For less pressing questions, leave a message in the general voicemail at either of our offices and your call will be returned on the next business day. Do I need to come back after my surgery?
Your surgeon will inform you if a post-operative visit is needed. If so directed, contact your clinic office to schedule an appointment.
Who do I call for prescription refills?
Call your pharmacy for refills.They will contact your MHC office for approval. Allow at least 24 hours for the prescription to be refilled. MHC offices do not fill narcotic prescriptions after office hours, so please make requests for your medications during our weekday hours of 8 am to 5 pm.
If your initial prescription was filled at the hospital following your surgery, you will need to contact your MHC office to refill your prescription. If you feel you need immediate medical attention, call 911.
