Billing and Insurance Information

Insurance
We participate with many HMO's, PPO's and Care Systems, and we will do everything we can to help you get the most from your medical insurance. A pre-registration representative will call you before your first appointment to collect your insurance information and pre-register you. Please bring your insurance cards to your first appointment.

Coverage concerns
Be sure to check your plan coverage and network listings before your visit. If your policy requires a referral from your primary care physician, make sure you get that authorization or your insurance company may refuse to pay for your care. Failure to obtain an authorization could mean that you would be responsible for payment of your medical costs. It is also crucial that you verify that the hospital location where your surgery will be done is in your insurance company’s network and that your policy covers the specific type of surgery you are having.

Payment
Co-payment Typically co-payments must be made at the time of service. If your coverage requires a co-payment please bring a means of payment (cash, check, MasterCard or Visa) to your appointment or a service charge may be added to your account.

Estimated costs
If you need surgery, our Administrative Office can provide you with an estimate of your costs. (Since coverages vary, the figure may not be precise.)

Claims filing
Our Administrative Office will file a claim with your insurance carrier. We will work with you to obtain the benefits you deserve; but, remember, you are accountable for the “patient responsibility” portion of our charges.

Financing
We do offer financing options to cover your portion of the cost, and we accept cash, checks, MasterCard or Visa. In some cases, you may be asked to pay for all or a portion of your surgery in advance. We will notify you if prepayment for your surgery is required.

Medicare
We also participate in the Medicare program and our Administrative Office can submit your claim to Medicare and any supplemental insurance carriers.
